Population health management refers to ways of bringing together health-related data to identify a specific population that health services may then prioritise. But with so many sources of relevant data, and the continuous increase in population what examples of prevention and population health management are we seeing today in Greater Manchester, the North and beyond?

This month's Health 2.0 meetup will explore a few examples of how digital technology is changing and helping population health management.
